Breeder/supplier/user authorisation applications (including amendments and renewals)

If you are intending to breed, supply and/or use animals for scientific or educational purposes, you must first be vetted by us and hold a breeder/supplier/user authorisation.  Below is a list of the forms and guides that are required for an application, amendment or withdrawal of authorisation.
