Individual authorisation applications (including amendments, renewals and reactivations)
Persons carrying out scientific procedures on animals or acting as a project manager for a project authorisation or who will be performing procedures or euthanasia of animals, must be authorised by the HPRA. Below is a list of the forms and guides that are required for an application, renewal, amendment or withdrawal of individual authorisations.

Retrospective assessment
Some project authorisations will be granted by the HPRA only on condition that a retrospective assessment be conducted following completion of the study. In order to submit the retrospective assessment, the below guide and form should be used:
