Medical Carbon Dioxide – PA 208/006/001
We wish to advise you that a number of batches of Medical Carbon Dioxide (C and E Size cylinders - 201-C and 201-E) are being recalled with immediate effect.
The reason for the recall is the potential for cylinders to be out of specification for carbon dioxide content, due to contamination with atmospheric air.

Actions to be taken
Information for healthcare professionals:
Healthcare professionals are requested to carry out the following actions:
1. Immediately quarantine any cylinders from the above listed batches in your possession. This includes all stock on wards, clinics, or any other locations within your facility.
2. Tag the quarantined cylinders with a BOC faulty cylinder tag, to identify the affected cylinders for collection.
3. If you have supplied any other pharmacy, clinic, or any other facility with units from the above-listed batches, please immediately forward a copy of the recall letter to them, requesting that they quarantine and return any units of the impacted batches to you.
4. Inform BOC of the quantity of cylinders from the above listed batches in your possession by contacting the Customer Service Centre on telephone no. 1800 355 255. Please also complete the Customer Return Form in the recall letter and email this to Michael.Shannon@boc.com. The collection of affected stock will then be arranged.
5. Quarantined stock should be notified to BOC by Friday 29th March 2024 in order to receive credit or replacement stock.
