Hospital Blood Bank Annual Report (HBBAR)

Regulation 12 of S.I No. 360 of 2005, European Communities (Quality and Safety of Human Blood and Blood Components) Regulations, requires that hospital blood banks submit an annual report to the Health Products Regulatory Authority (HPRA). Hospital blood banks are required to submit a declaration of compliance with the requirements of S.I. No. 360 of 2005 and provide details of the systems that are in place to ensure such compliance.

The Hospital Blood Bank Annual Report form has been developed by the team of inspectors within the HPRA that is responsible for Blood and Tissues.  The report forms are available on the HPRA Website.  Hospital Blood Banks are requested to return the completed forms to the HPRA by 1st March of each year.
